The nasal sept is the wall that divides the nose into the passages of the left and right nasal airway. The nasal sept is composed of cartilage and thin bone that is covered on both sides by a thin layer of soft tissue. You can feel the nasal sept as the middle structure when the tip of the nose is pinched. A nasal perforation is a hole through the nasal sept. The hole can vary in size depending on the cause of the perforation. The following illustrates the causes, symptoms and final treatments for septal nasal perforation repair when necessary. What are the symptoms of a septal perforation? Septal perforations cause disorders in how the air moves through the nasal passages, making the airflow more turbulent and less smooth. The air passes from one side to the other side of the nose instead of returning directly to the airway. As a result, having a septal perforation can cause you to experience some of the following symptoms, if not all: Causes of a nasal septal perforation Nose septal perforations are caused by the interruption of blood flow to the nasal septal. Because the nasal septo has a delicate blood supply, any factor that decreases this blood flow can result in a septal perforation. Some of the most common causes of septal perforations are: Because of the complex and potentially interconnected causes of a nasal septal perforation, it is important to talk to a specialist. Can a septal perforation heal in your own? Sometimes, depending on the size of the septal perforation, you can heal on your own. However, as the size increases, the probability of spontaneous resolution decreases. A proper repair of nasal septal perforation is important due to the delicate nature of the tissue. What makes someone a specialist in repairing the Nasal Seal drilling? Specific surgeons are more equipped to handle and treat septal perforations. Typically, otolaryngologists (or ENT surgeons) have the skills to repair such septal perforations. The most complex and large perforations may require specialists such as facial plastic surgeons to repair the defect. Wherever the nasal septal perforation repair is done, make sure you feel comfortable and confident in the specialist you work with. How are the Nasal Separate Perforations treated? The objectives of septal drilling management are to reduce the annoying symptoms resulting from nasal dryness. Therefore, management involves measures that are designed to increase the humidification of the nasal air that passes, to prevent nasal crucisting, bleeding, staging and obstruction of the airways. Nasal creams such as a vaseline ointment or antibacterial ointment applied to nasal pits are designed to prevent the drying of the tissue. Night dryness can be a problem, especially while sleeping. Therefore, we can often recommend the use of a bed humidifier. The use of saline nasal irrigation can also help to wash the crusting, and prevent the accumulation of crusts by maintaining a wet environment. However, such measures are limited as they are designed to address only the symptoms and not to correct the perforation itself. For persistently annoying drilling, there are surgical options. Preparation for North Perforation RepairThe perforations that have the symptoms including bleeding, gnashing, cloaking, or nasal collapse should be evaluated for surgical intervention when the symptoms become onerous. The success of repairs depends on maintaining good blood flow in the postoperative period. Patients are encouraged to quit for at least 6 weeks before and after 2 weeks after surgery. In addition, patients may need to discontinue supplements or counter-restricted medications that may harm healing. How is a septal perforation repaired? Depending on the size of the perforation, and its exact location on the nose, there are several surgical options for repair: Septal Button This type of repair involves placing a prosthesis to plug everything. Such repair may be annoying since a foreign object is placed in the nose. This can also cause infections and require frequent visits to the doctor for a replacement. These have fallen out of favor by most doctors and patients. Local Flap For small perforations (less than 2.5 cm) can be used to repair drilling. This type of repair uses neighboring tissue in the perforation region to seal the hole and cover it. This procedure usually involves the same day surgery in an outpatient surgery center. After the operation, you may experience some discomfort during the healing process. As the drilling size increases, the success rate of a local flap decreases. However, this type of repair is less symptomatic for patients than using a septal button as no foreign material is placed on the nose that needs to be replaced, or may be infected. Regional Flap For drilling (2.5 – 3.5cm), it may not be enough to close the drilling. Other options include the use of face or mouth tissue to cover and seal the hole. Free Flap For a larger perforation (with 3.5 cm), a transplanted scalp tissue can be made and used to reconstruct the missing tissue. After surgery and recoveryDepending on the type of surgery you have, you are likely to have nasal packing/packages on your nose. These are placed by Dr. Mourad to allow the proper healing of the inner nasal structures, as an "inner pussy" for the nose. These are usually eliminated on your first postoperative visit, a week later. It is also likely that you have a nasal plaster outside your nose. This also helps to reduce swelling and keep everything in place. This stays in the nose also for 1 week after surgery. Depending on your work, Dr. Mourad usually recommends taking a minimum of 3 days off before returning to work. It also discourages your patients from any heavy lifting, as this can alter the nose structure by stretching. If you wear glasses, you may need some alterations that do not press on the nasal bones. Dr. Mourad will provide you with pain medications, ointments, nasal sprays, all to help maximize your results. You'll see Dr. Mourad a week after surgery where he will remove all melting materials and fragments. Your nose may be swollen for a temporary period of time, so don't stress! The nose is a small area, and everything that surgery causes inflammation that eventually comes down over time. Depending on the patient, this inflammation usually lasts 2-3 weeks but can last up to two months in rare circumstances. To help reduce swelling, Dr. Mourad will reapply the tape to the nose that will be there until his second postoperative visit. This pressure dressing reduces the amount of inflammation. The second postoperative visit will involve the removal of the entire tape. Dr. Mourad will see you on visits two weeks later, and then a month later.
